AgeWell PACE, a non-profit collaboration between LifeLong Medical Care and Santa Rosa Community Health, provides comprehensive health and social services for seniors in Marin and Sonoma Counties. AgeWell PACE opened its first center in Rohnert Park in 2023, offering a one-of-a-kind support system for older adults in the region. Whether you are a senior, caregiver, or community member, this event will provide valuable insights into how AgeWell PACE can help you or your loved ones maintain independence and well-being.

Susana Avila started at AgeWell PACE in March of 2023, as the third employee hired. AgeWell PACE opened its doors later that year in July. She started as the Executive Assistant to the Executive Director and a year later was moved into the Community Outreach role. Her knowledge of the PACE program makes her the best AgeWell PACE cheerleader and the best to be out in the community presenting on the services AgeWell PACE offers. Susana has been a part of PACE since the beginning, seeing it come full circle and into fruition and it really shows in her presentations. She wore many hats at the beginning but has grown into the Community Outreach position with such love and care. Her background in project management has made her work very systematically in her roles and her love for the program has allowed her to excel in her new role.
